How to Write a PICOT Question

PICOT is the nursing and evidence-based-practice variant of the more general PICO framework — if you're writing a question for a literature-review-style systematic review, see our general PICO question guide instead. PICOT adds a "T" for timeframe, and it's built specifically for framing a practice-change question, not a review question.

The five elements

• Population — the specific patient group on your unit or in your setting.
• Intervention — the practice change you're proposing.
• Comparison — current practice or standard of care.
• Outcome — the specific, measurable clinical or operational result.
• Timeframe — how long the intervention runs before the outcome is measured.

Worked examples by specialty

• Med-surg: In post-surgical adult patients (P), does implementing a structured early-mobility protocol (I), compared to standard mobility orders (C), reduce hospital-acquired pressure injuries (O) within a 30-day period (T)?
• ICU: In mechanically ventilated ICU patients (P), does a nurse-driven sedation protocol (I), compared to physician-directed sedation (C), reduce ventilator days (O) over a 3-month implementation period (T)?
• Pediatrics: In pediatric patients undergoing IV insertion (P), does topical anesthetic application (I), compared to no topical anesthetic (C), reduce procedural pain scores (O) measured immediately post-insertion (T)?
• Psychiatric/behavioral health: In adult inpatients on a behavioral health unit (P), does a structured de-escalation training program for staff (I), compared to no formal training (C), reduce the rate of physical restraint use (O) over a 6-month period (T)?

Common mistakes

• Skipping the timeframe, or leaving it too vague to actually evaluate ("eventually" isn't a timeframe).
• An outcome that isn't something your unit can realistically measure with the data access you have.
• A question phrased as a yes/no research hypothesis instead of a specific, answerable practice question.
